The following procedures must be followed before submitting an exclusion request in the automated system:
- Officially notify the trainee of the observation or violation, ensuring the notification includes (the observation or violation, trainee details, notification date, and is signed and stamped by the organization), and give the trainee an opportunity for three written notifications, ensuring they are not issued in close succession.
- If the trainee does not respond, the trainee should be formally directed to withdraw from the training opportunity by the organization.
